In a small saucepan, whisk together 1/2 cup of half and half and the cocoa powder. In the video i did all this together, but after making again it mixes much easier with 1/2 cup at first.
Heat on low, whisking continuously until combined (about 3 minutes).
Then add in sweetened condensed milk and combine well.
Remove from heat and whisk in remaining ingredients. (extracts) Pour into a quart sized mason jar, with lid.
Make sure to shake vigorously before serving

I love half and half for this creamer, but feel free to use any milk. Need this dairy free? Feel free to substitute with milk almond milk, cashew or your choice of non dairy. Store this creamer in the fridge with airtight container. Shake before each use. This creamer will keep for up to the expiration date on the milk.
